dc.description.abstractThe study was on utilization of long acting reversible contraceptives among commercial sex workers aged 15 - 49 years in Nairobi county slums. Long acting reversible contraceptives methods are intrauterine devices and implants. The failure rate is quite low, hence friendly to most women who are sexually active. Specific objective was to establish the types of long- acting reversible contraceptives used among commercial sex workers, determine service provision points, association of socio-demographic characteristic and challenges faced in accessing. A descriptive cross-sectional study design where both quantitative and qualitative data collection methods were applied. The study area was Kibra slums of Nairobi, Kenya. The target population was commercial sex workers aged 15 to 49 years of age. The sample size was derived from the Nairobi population of commercial sex workers which has 27,000. Using Fishers et al formula it was calculated to be 384. Simple random sampling technique was used to select 8 out of the 13 villages in Kibra. Purposive sampling was used to select 24 hotspots out of 35 while snowballing sampling was used to recruit the participants. Structured questionnaire and interview guides were used to collect data. Ethical consideration was put in place. The study revealed that there is low utilization of long acting reversible contraceptives among commercial sex workers 15 to 49 years, where 41% utilized while 59% did not utilize. This study found out that 32% obtain their contraceptives from the chemists while 31% from the Public Health facilities they preferred the chemists to avoid long ques in the public facilities. The study established that 35% of the sex workers were between the ages of 15-24 years, which indicated that they start sex worker at a young age, and if intervention of birth control are not put in place, then this is a segment of the population that can end up having unintended pregnancies. The study further noted that 73% had attained primary education and hence poor knowledge on long acting reversible contraceptives compared to tertiary level. The findings revealed that there was strong statistical significant association between the age χ2 = 188.230a , df =3, p = <0.001,level of education χ2 = 211.058a , df = 3, P =<0.001 and marital status, χ2 = 240.747a , df = 3, P = <0.001 in utilization of long-acting reversible contraceptives. The religion and utilization of long-acting reversible contraceptives, the study showed there was no statistically significant association χ2 = 5.102a, df = 3, p = < 0. 164.The challenges faced by commercial sex workers include myth and misconceptions, client demanding a baby, out of stock The Policy makers to develop relevant policies and guidelines to support utilization of long -acting reversible contraceptives among the women of reproductive age. The Community Health Assistants and Peer Educators need to create awareness on utilization of long- acting reversible contraceptives in the community and outreach services enhanced. Commercial sex workers to adopt a benefit-behavior that reduces the possibility of having negative outcomes following unintended pregnancy. Similar studies to be carried out in other socio-economical settings and geographical locations.en_US
